- want to change any registered account details e.g. phone number, postal address, first name, or last name
- have questions about your medical record
If you have not been able to find the answer you are looking for in our support pages and experiencing problems with the App, please contact our support team by either:
- Clicking SUPPORT on the top right of the app
- Note: If you are logged in it will automatically fill in your name and email saving you time
- Visit https://support.patientpack.co.uk/hc/en-us/requests/new and choose "I have an issue with using the app" from the drop-down list
To help resolve your support request promptly, please confirm that you have carried out these preliminary checks:
- Cleared the temporary internet files (cache) on your device
- Uninstalled the App, then reinstalled the App
- Restarted your device
- Logged into the App and re-tested
If you still experience problems, please provide as much of the following information as possible:
- Which device are you using?
e.g. PC/Mac/Android Phone/iPhone
- Which internet connection are you using?
e.g. WiFi/Ethernet/Mobile network
- When you get the error, what options had you selected beforehand?
e.g. Login (using NHS Login) > My Record > Conditions > Current
- If possible, please provide screenshot and describe the problem in your own words?
- If you are using a PC or Mac, please provide a copy of the URL?
