If you have changed your GP, we will also need to change this on the App.
To do this please contact us by either:
- Clicking SUPPORT on the top right of the App
- Visiting https://support.patientpack.co.uk/hc/en-us/requests/new and then choose "I have an issue with using the app" from the drop-down list

- Old GP:
- New GP:
- When did you change GP?
- Has your new GP confirmed that you are now registered on their clinical systems/records? Yes/No
Note: We can't provide timescales of when this amendment will be completed, as we need to liaise with your new GP to confirm some details.
