The most common causes of this error are:
- that your GP surgery has decided to no longer use this application and their service for this App has been turned off
- while applying for your NHS login, our App was not able to identify your GP surgery correctly
- you might have recently changed your GP surgery
- your account might have been deactivated by your surgery
